General Information

This form should be completed for Special Waste approval only. Special waste is a waste that requires special handling or has potential contaminants. Examples include asbestos, RV and mobile homes, dead animals, industrial process wastes, incinerator ash, off-spec products, treated biomedical waste and empty chemical containers. Sampling Criteria

Some industrial/commercial wastes require analytical testing to determine if they are acceptable for disposal in the Solid Waste Management System. The Hillsborough County Solid Waste Management Division (SWMD) may require additional information on your waste stream. The SWMD reserves the right to require additional analysis of the waste.

  • This waste is not hazardous waste (as defined by the USEPA 40 CFR Part 260.10) Federal Regulation or other State and Local Regulations.
  • This waste does not contain any levels of Polychlorinated Biphenyls (PCBs).
  • This waste does not contain any infectious, biomedical or biohazardous waste materials.
  • This form contains a true and accurate description of the waste material to be disposed.
  • All relevant information regarding known or suspect hazards in possession of the generator has been disclosed.
